FLORIDA DEPARTMENT OF CORRECTIONS

FDC Assault Advisories | July 6 – July 12, 2022

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Hernando Correctional Institution: On July 6, Inmate Rayshod Weaver (E41571) and Inmate Ojay Nish (M37880) assaulted an officer by falling on them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Walton Correctional Institution: On July 6, Inmate Anthony Horsley (E44830) assaulted officers by pulling the inside of their mouths and kicking them in their sides. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officers and noted injuries.

Martin Correctional Institution: On July 6, Inmate Chazelle Reed (C00675) assaulted an officer by punching and biting them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Charlotte Correctional Institution: On July 6, Inmate Dwayne Mitchel (E20021) assaulted an officer by kicking their leg. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Annex: On July 7, Inmate Robert Culver (J52980) assaulted officers by striking them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officers and noted injuries.

Martin Correctional Institution: On July 7, Inmate Terrell Taylor (J44569) assaulted officers by striking them with hand cuffs. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officers and noted injuries.

Florida State Prison: On July 7, Inmate Joshua Clark (135082) assaulted an officer by kissing the officer. Staff responded appropriately, and the inmate was subdued.

Franklin Correctional Institution: On July 7, Inmate Donald Goins (J07268) assaulted a staff member by pushing them in the chest . Staff responded appropriately, and the inmate was subdued.

Florida State Prison: On July 7, Inmate Remoi Campbell (Y62445) assaulted an officer by officer kicking and biting them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Holmes Correctional Institution: On July 8, Inmate Jacob Hawkins (536856) assaulted an officer by grabbing their sleeve. Staff responded appropriately, and the inmate was subdued.

Jefferson Correctional Institution: On July 8, Inmate Blake Guild (Y49578) assaulted an officer by striking them with a lock in a canteen bag. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officer and noted injuries.

Suwannee Correctional Institution: On July 8, Inmate Shauntwan Barnes (N21938) assaulted an officer by spitting on them. Staff responded appropriately, and the inmate was subdued.

Central Florida Reception Center: On July 8, Inmate Brad Helms (W90440) assaulted an officer by striking them in their ribs. Staff responded appropriately, and the inmate was subdued.

Okaloosa Correctional Institution: On July 8, Inmate Scott Tarase (311802) assaulted officers by pulling and spitting on them. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On July 8, Inmate Odell Lee (K79438) assaulted officers by striking them. Staff responded appropriately, and the inmate was subdued.

Charlotte Correctional Institution: On July 9, Inmate Zebelede Garrison (B12696) assaulted an officer by spitting them. Staff responded appropriately, and the inmate was subdued.

Okeechobee Correctional Institution: On July 9, Inmate Micheal Allen (L74760) assaulted an officer by pushing them in their chest. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On July 10, Inmate Carlton Matthews (J27519) assaulted an officer by spitting in their face. Staff responded appropriately, and the inmate was subdued.

Walton Correctional Institution: On July 10, Inmate Dekevious Menefee (E42893) assaulted an officer by striking their arm. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On July 10, Inmate John Cole (438121) assaulted officers by scratching them. Staff responded appropriately, and the inmate was subdued. Medical staff examined the officers and noted injuries.

Suwannee Correctional Institution: On July 11, Inmate Jose Martins (Y58512) assaulted an officer by spitting in their face . Staff responded appropriately, and the inmate was subdued.

Apalachee Correctional Institution: On July 12, Inmate Marques Kirk (B09042) assaulted an officer by grabbing them. Staff responded appropriately, and the inmate was subdued.

Northwest Florida Reception Center: On July 12, Inmate Jontae Starks (Q40434) assaulted an officer by striking on their arm. Staff responded appropriately, and the inmate was subdued.

Charlotte Correctional Institution: On July 12, Inmate James Cochran (G11719) assaulted an officer by spitting their face. Staff responded appropriately, and the inmate was subdued.

Northwest Florida Reception Center: On July 12, Inmate Thomas Schmitt (838180) assaulted a staff member by grabbing them by the shoulders. Staff responded appropriately, and the inmate was subdued.

Lowell Correctional Institution: On July 12, Inmate Nicole Zamano (170711) assaulted an officer by pushing them. Staff responded appropriately, and the inmate was subdued.
