FDC Assault Advisories | April 19 - April 25, 2023
This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.
Avon Park Correctional Institution: On April 19, Inmate Courtenay Johnson (P71369) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Lake Correctional Institution: On April 19, Inmate John Hartman (B81742) assaulted officers by biting them. Staff responded appropriately, and the inmate was subdued. Medical staff recommended outside medical evaluation. The officer was sent to receive outside medical treatment.
Florida Women's Reception Center: On April 19, Inmate Christina Maxwell (A70008) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Okeechobee Correctional Institution: On April 19, Inmate Efrim Chappell (328436) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Hardee Correctional Institution: On April 19, Inmate Karl Castro (V28112) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Reception & Medical Center: On April 19, Inmate Jerry Jackson (X45682) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Desoto Annex: On April 20, Inmate Patrick Zalewski (L37926) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Florida Women's Reception Center: On April 20, Inmate Amber Sills (K76345) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Jackson Correctional Institution: On April 20, Inmate Kamal Williams (L83540) assaulted a staff member. Staff responded appropriately, and the inmate was subdued.
Santa Rosa Correctional Institution: On April 20, Inmate John Croissy (M46994) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Sumter Workcamp: On April 20, Inmate Brandon Koehler (T93273) assaulted officers. Staff responded appropriately, and the inmate was subdued.
Santa Rosa Annex: On April 20, Inmate Christopher Thornton (134213) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Central Florida Reception Center: On April 20, Inmate Anthony Smith (312505) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Martin Correctional Institution: On April 20, Inmate Oscar Gonzalez (192123) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Northwest Florida Reception Center: On April 20, Inmate Trallas White (T39882) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Hardee Correctional Institution: On April 20, Inmate Eddie Wright (L84828) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Florida State Prison: On April 20, Inmate Elford Brown (P71648) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Central Florida Reception Center: On April 21, Inmate Randy Crosby (S17435) assaulted officers. Staff responded appropriately, and the inmate was subdued.
Northwest Florida Reception Center: On April 21, Inmate Lenford Gibbons (L93216) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Calhoun Correctional Institution: On April 22, Inmate Terrance Jones (T89424) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Lake Correctional Institution: On April 22, Inmate Tyrone Mitchell (111551) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Northwest Florida Reception Center: On April 22, Inmate Juan Coliazo (W81090) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Florida State Prison: On April 23, Inmate Le Darius Smith (J58075) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Northwest Florida Reception Center: On April 23, Inmate Juan Coliazo (W81090) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Franklin Correctional Institution: On April 23, Inmate Johnny Johnson (C70116) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Florida State Prison: On April 23, Inmate Antwon Anderson (C05253) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Lowell Correctional Institution: On April 24, Inmate Erica Abraham (A10188) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Florida State Prison: On April 24, Inmate Jamal Vass (B14084) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Dade Correctional Institution: On April 24, Inmate Lewis Smith (B07953) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Santa Rosa Correctional Institution: On April 25, Inmate Willie Lightfoot (663180) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
Avon Park Workcamp: On April 25, Inmate Ismael Sylvain (L59375) assaulted officers. Staff responded appropriately, and the inmate was subdued.
Florida State Prison: On April 25, Inmate Ronald Green (C02017) assaulted officers. Staff responded appropriately, and the inmate was subdued.
Santa Rosa Correctional Institution: On April 25, Inmate Anthony Leeman (Q90159) assaulted an officer. Staff responded appropriately, and the inmate was subdued.
