Instructions for Program Implementation Checklist (PIC):
The Program Implementation Checklist (PIC) is a form designed to assist Wardens at Female Institutions with implementing programs in their institutions. It is also designed to help the Female Offender Program Unit and the department report on the status of specific programs at female offender institutions. Wardens should submit a PIC for each program being implemented.
DIRECTIONS: Wardens should provide the information requested at the top of the form: the name of the institution at which the program will be implemented, the name of the program to be implemented, and the start date and target completion date of the program. As new programs are accepted and/or chosen, Wardens are to complete and submit a copy of a Program Implementation Checklist (PIC) for each program.
Each duty listed on the PIC is the responsibility of the Warden. The Warden should therefore delegate each duty accordingly and ensure that the task has been completed by assigned staff. When each duty has been completed, the Warden should check the appropriate space. If a duty was not performed, the Warden should explain reasons for non-compliance and attach this explanation to the PIC for submission.
By the target date for completion of each program, the Warden should submit a signed copy of the PIC and any comments to the Administrator for Female Offenders, Bureau of Special Institutional Services, Office of Program Services, Florida Department of Corrections, 2601 Blair Stone Road, Tallahassee, FL 32399-2500.
