FDC Assault Advisories | October 11–17, 2016

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty.

CLERMONT, Fla. – On October 11, an inmate assaulted a correctional officer at Lake Correctional Institution.
Inmate Timothy Skeens (T63029) attacked the correctional officer at approximately 3:20 p.m. EST. The officer was returning Inmate Skeens to his cell when the inmate turned and kicked the officer. Additional staff responded appropriately and the inmate became compliant.

Medical staff examined the officer and noted no injuries.

Inmate Skeens will receive a disciplinary report for this assault.


RAIFORD, Fla. – On October 11, an inmate assaulted a correctional officer at Florida State Prison.
Inmate Jimmy Hargraves (D47617) attacked the correctional officer at approximately 8:35 p.m. EST. Inmate Hargraves was acting unresponsive in his cell. When staff entered the cell to check on him, the inmate struck the officer. Staff responded appropriately, and the inmate was restrained.

Medical staff examined the officer and noted no injuries.

Inmate Hargraves will receive a disciplinary report for this assault.


MALONE, Fla. – On October 12, an inmate assaulted a correctional officer at Jackson Correctional Institution.
Inmate Chadrick Thomas (T32321) attacked the correctional officer at approximately 2:58 p.m. CT. Inmate Thomas was causing a disturbance in his cell. As staff attempted to cease the inmate’s actions, he kicked the correctional officer. Staff responded appropriately, and there was no further incident.

Medical staff examined the injured officer and noted a scratch on a finger.

Inmate Thomas will receive a disciplinary report for this assault.


LAKE CITY, Fla. – On October 13, a correctional officer was assaulted by an inmate at the Columbia Annex.
Inmate Damon White (K57566) attacked the correctional officer at approximately 2:20 p.m. EST. Inmate White became non-compliant while trying to get rid of a homemade weapon and kicked the officer. Staff responded appropriately, and the inmate became compliant.

Medical staff examined the injured officer and noted a laceration to the back of a hand.

Inmate White will receive a disciplinary report for this assault.


LAKE BUTLER, Fla. – On October 13, an inmate assaulted a correctional officer at Reception and Medical Center.
Inmate Johnathan Harris (M66821) attacked the correctional officer at approximately 5:15 p.m. EST. The officer was speaking with Inmate Harris when the inmate struck the officer in the face. Additional staff responded appropriately, and the inmate became compliant.

Medical staff examined the injured officer and noted a scratch to the head and a hematoma to the cheek.

Inmate Harris will receive a disciplinary report for this assault.


INDIANTOWN, Fla. – On October 13, an inmate assaulted a correctional officer at Martin Correctional Institution.
Inmate Chadd Smith (R29176) attacked the correctional officer at approximately 7:10 p.m. EST. Inmate Smith was being escorted when he pulled away and head butted the officer. Staff responded appropriately, and the inmate became compliant.

Medical staff examined the officer and noted no injury.

Inmate Smith will receive a disciplinary report for this assault.


CLERMONT, Fla. – On October 14, an inmate assaulted a correctional officer at Lake Correctional Institution.
Inmate Terrell Forrest (Q26038) attacked the correctional officer at approximately 11:25 a.m. EST. Inmate Forrest struck the officer in the face and neck area. Staff responded appropriately, and the inmate became compliant.

Medical staff examined the injured officer and noted redness to the lip and neck.

Inmate Forrest will receive a disciplinary report for this assault.


MILTON, Fla. – On October 14, an inmate assaulted correctional officers at Santa Rosa Correctional Institution.
Inmate Darryl Black (128690) attacked the correctional officers at approximately 6:00 p.m. CT. As he was being removed from the shower, Inmate Black slipped his restraints and assaulted the officers with a weapon. Additional staff responded appropriately, and the inmate became compliant.

Medical staff examined the injured officers and noted the following: three puncture wounds to the abdomen, one puncture wound to the rib cage and one puncture wound to an arm.

Inmate Black will receive a disciplinary report for this assault.


WEWAHITCHKA, Fla. – On October 16, an inmate assaulted correctional officers at Gulf Correctional Institution.
Inmate Nathan Jackson (P01715) attacked the correctional officer at approximately 6:25 a.m. CT. The inmate was in the process of being searched when he pushed the officer’s head into a fence post. They went to the ground and Inmate Boyll then began striking the officer’s head and upper torso. Additional staff responded appropriately, and the inmate became compliant.

Medical staff examined the injured officer and noted abrasions on the head, elbows and the back of an ear.

Inmate Jackson will receive a disciplinary report for this assault.


CENTURY, Fla. – On October 17, an inmate assaulted a correctional officer at Century Correctional Institution.
Inmate Elijah Union (468070) attacked the correctional officer at approximately 8:30 a.m. CT. Inmate Union entered an unauthorized area and knocked over a water cooler. After ignoring orders cease his actions, the inmate began striking the officer in the face. Additional staff responded appropriately, and the inmate became compliant.

Medical staff examined the injured officer and noted scratches on a forearm, scratches and abrasions to the face, and a laceration under an eye.

Inmate Union will receive a disciplinary report for this assault.


*JASPER, Fla. – On October 17, an inmate assaulted a correctional officer at Hamilton Annex.
Inmate Chester Hoffman (H45384) attacked the correctional officer at approximately 3:19 p.m. EST. While exiting his cell, Inmate Hoffman swung his elbow and struck the officer in the torso. Staff responded appropriately, and the inmate was restrained.

Medical staff examined the officer and noted no injuries.

Inmate Hoffman will receive a disciplinary report for this assault.


*MADISON, Fla. – On October 17, an inmate assaulted a correctional officer at Madison Correctional Institution.
Inmate Brandon Jones (E41313) attacked the correctional officer at approximately 3:55 p.m. EST. Inmate Jones ran toward and struck the officer in the upper torso. The inmate knocked the officer to the ground and continued to strike the head of the officer. Staff responded appropriately, and the inmate was restrained.

Medical staff examined the officer and noted soreness to the torso and head.

Inmate Jones will receive a disciplinary report for this assault.
