FDC Assault Advisories | December 20 - 26, 2016

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty.

CRESTVIEW, Fla. – On December 20, an inmate assaulted a correctional officer at Okaloosa Correctional Institution.

Inmate Alexander Ortiz (425491) attacked the officer at approximately 3:05 p.m. CDT. During a search, Inmate Ortiz charged at and began beating the officer. Staff responded appropriately to subdue the inmate.

Medical staff examined the employee and noted injuries. Inmate Ortiz will receive a disciplinary report for this assault.


LIVE OAK, Fla. – On December 22, an inmate assaulted a correctional officer at Suwannee Correctional Institution.

Inmate Patrick Deboard (L25900) attacked the officer at approximately 8:30 a.m. EDT. Inmate Deboard struck the officer in the facial area. The inmate became compliant without further incident.

Medical staff examined the employee and noted an abrasion on the lip. Inmate Deboard will receive a disciplinary report for this assault.


DAYTONA BEACH, Fla. – On December 22, an inmate assaulted a correctional officer at Tomoka Correctional Institution.

Inmate Dekevious Burton (C04293) attacked the officer at approximately 8:55 a.m. EDT. Inmate Burton was refusing orders during transport when he became combative. Staff responded appropriately, and the inmate became compliant.

Medical staff examined the employee and noted abrasions on the hand, face and back. Inmate Burton will receive a disciplinary report for this assault.


PUNTA GORDA, Fla. – On December 24, an inmate assaulted a correctional officer at Charlotte Correctional Institution.

Inmate William Fleming (J41571) attacked the officer at approximately 7:20 a.m. EDT. Inmate Fleming struck the officer in the face with a closed fist. Staff responded appropriately, and the inmate became compliant.

Inmate Fleming will receive a disciplinary report for this assault.


CLERMONT, Fla. – On December 25, an inmate assaulted a correctional officer at Lake Correctional Institution.

Inmate Charles Wade (X29271) attacked the officer at approximately 4:40 a.m. EDT. Inmate Wade was in escort when he head butted the officer in the face. Staff responded appropriately, and the inmate became compliant.

Medical Staff examined the employee and noted bruising and swelling to the eye. Inmate Wade will receive a disciplinary report for this assault.


CHIPLEY, Fla. – On December 26, an inmate assaulted a correctional officer at Northwest Florida Reception Center Annex.

Inmate Julio Catala Nunez (X88245) attacked the officer at approximately 9:45 a.m. CDT. Inmate Catala Nunez struck the officer in the hand and knee. The inmate became compliant without further incident.

Medical staff examined the employee and noted injuries. Inmate Catala Nunez will receive a disciplinary report for this assault.


LIVE OAK, Fla. – On December 26, an inmate assaulted a correctional officer at Suwannee Correctional Institution.

Inmate Eddie Murphy (I12160) attacked the officer at approximately 8:10 p.m. EDT. Inmate Murphy grabbed the arm of the officer and caused injury as the officer tried to pull away. Staff responded appropriately.

Medical staff examined the employee and noted a swollen finger. Inmate Murphy will receive a disciplinary report for this assault.
